Represent your communityThis is a very important survey conducted every three years. Results from the survey will be used to help local health and human service organizations make Weld County a healthier place to live, learn, work, and play. By understanding the health needs of residents, organizations can do a better job providing services and improving the well-being of residents. Previous survey results have been used to help improve the environment and quality of life for residents.

Our community health survey asks about physical and mental health conditions, habits, and behaviors, health insurance including barriers to accessing health care, opinions about environmental health issues, and needs related to health and well-being.

The survey is available in both English and Spanish. If you received a survey in the mail, you do have the option to complete it online.

If you did not receive a survey in the mail but would like to participate in our assessment process, contact Audrey Goold at to be put on the list to take part in one of our upcoming focus groups, community forums, or meetings.
