Providing regular, updated data on the health of Weld County's population is a core function of the Weld County Department of Public Health and Environment. Data is intended to inform the community, including policy makers, medical personnel, researchers, community-based organizations, government agencies, and individuals to understand and respond to diseases, threats, and underlying conditions affecting our health.

The aim of Thriving Weld County’s Community Health Improvement Plan (CHIP) is two-fold: 1) to reduce people's risk for chronic disease and help them maintain a healthy body weight and 2) to increase years of healthy life and reduce disparities among all Weld County residents.
