Immunizations For Children
The Weld County Health Department provides routine immunizations for children, including immunizations required by Colorado School Law. Immunizations are available for children 0-18 years of age through the Vaccine for Children program. Community education and immunization resources also are available. If you are unable to make your appointment, please call to cancel so it is available for someone else.

A parent or legal guardian must be present for all children under the age of 18 years old or a signed Weld County Health Department consent form must be filled out prior to the appointment.

Payment is requested at time of service. Cash, check, and credit cards are accepted. Private insurance is accepted. Additional fees do apply. See immunization fees. No child is turned away for routine vaccines due to inability to pay.

If you are Medicaid eligible, please bring Medicaid card to visit.

Please bring all past immunization records. Child will not be seen without past immunization records (unless prior authorization is granted.)
