Immunizations For Children
The Weld County Health Department provides routine immunizations for children, including immunizations required by Colorado School Law. Immunizations are available for children 0-18 years of age through the Vaccine for Children program. Community education and immunization resources also are available. Call (970) 304-6420 if you would like more information.

If you have questions about a specific vaccine or would like to learn more about vaccines and immunizations in general, go to www.vaccineinformation.org.

If you are unable to make your appointment, please call to cancel so it is available for someone else.

A parent or legal guardian must be present for all children under the age of 18 years old or a signed Weld County Health Department consent form must be filled out prior to the appointment.

Payment is requested at time of service. Cash, check, and credit cards are accepted. Private insurance is accepted. Additional fees do apply. See immunization fees. No child is turned away for routine vaccines due to inability to pay.

If you are Medicaid eligible, please bring your Medicaid card to your visit.

Please bring all past immunization records. Children will not be seen without past immunization records (unless prior authorization is granted.)

On the 2nd Monday of each month the hours are:
10:00 a.m. – noon; 1:00 p.m. – 7:00 p.m.
